This should be SQLTalk from 11.6SP1. When a command with bind variables is executed, pressing Cancel in the dialog will cause an unhandled exception:

Unhandled exception at 0x0115bb42 in sqltalk.exe: 0xC0000005: Access violation reading location 0x00000010.

Any select statement with bind variable should reproduce the problem:
SELECT NAME FROM SYSTABLES WHERE NAME=:1;
